Managing LTSS for Individuals With Complex Care Needs



High-need, high-costs patients are those with multiple or complex conditions, and are often combined with behavioral health problems or socioeconomic challenges. As the United States grapples with steeply rising health care costs, payers, providers, and policymakers are seeking ways to improve the efficiency of health care delivery for complex patients. Implementing and managing complex care management programs offers the opportunity to provide efficient and more coordinated care for patients while also reducing cost. The goals of complex case management are to maintain or improve a patient’s functional status, increase their capacity to self-manage their condition, eliminate unnecessary clinical testing, and reduce the need for acute care services. This summit will bring together provider, payer, and policy stakeholders to discuss best practices for implementing successful care management programs for complex populations.
