Engaging Individuals at Risk of Diabetes: Health Plan Outreach Strategies
As part of the AHIP national initiative in cooperation with the CDC, health plan leaders from Denver Health, EmblemHealth, Florida Blue, and Molina Healthcare will discuss multiple outreach strategies for engaging consumers at risk of diabetes type II through the implementation of the National Diabetes Prevention Program. Spanning Commercial and Medicaid covered individuals, urban and suburban settings, employer and insurer experiences, hear how health plans are reaching an incredibly diverse group.
Attendees will gain insights and perspective on:
- The components of the CDC National Diabetes Prevention Program, a one year lifestyle change program.
- Health plan strategies for engaging individuals at high risk for diabetes, specifically hosting sessions utilizing varied settings – workplace, community, provider, and retail centers.
- Use of multiple delivery models to demonstrate value and scalability through outreach and educational efforts around the National Diabetes Prevention Program in California, Florida, New Mexico, New York, and Wisconsin.
