Tennessee's Transition to a Single, Unified System of Care



Like many public sector agencies, the Tennessee Department of Health’s Early Childhood Section manages a lot of data. 

 

Their evidence-based home visiting programs capture data on families from across the state to guide their efforts in promoting the health and wellness of Tennessee’s youngest residents and their caregivers. But until recently, this valuable information – which is regularly leveraged by program directors, funders and state agencies – was still managed using spreadsheets. This resulted in unstandardized datasets, overlapping or duplicated reporting efforts and drained personnel hours.

 

By transitioning to single, unified case management system, the division is on track to make huge improvements to its workflows and interactions with constituents throughout the state.

Joana Rosales, Clinical Applications Coordinator and Epidemiologist, Tennessee Department of Health

Joana Rosales has been with the Tennessee Department of Health (TDH) for over 5 years. She is currently the Clinical Applications Coordinator (CAC) for the Early Childhood Initiatives Section at TDH. In this position, she oversees data reporting for all evidence based home visiting (EBHV) programs funded by Temporary Assistance for Needy Families (TANF), which is approximately half of the state EBHV system. Joana is also responsible for the acquisition and implementation of a new statewide database system for the EBHV system in Tennessee, and she provides support for the transition to a new state Electronic Health Record System.

Prior to this role, Joana was the Epidemiologist for the CHANT (Community Health Access and Navigation in Tennessee) statewide program at TDH. In this position, she assisted with the creation of a data entry platform; developed reporting measures for state funders; and analyzed data for reports and assisted in implementation and training.


Jill Guffey, Director of Solutions Architecture, Bonterra

Jill Guffey is a Director of Solutions Architecture with Bonterra where each day she and her teams enable and elevate the hardworking people at organizations that do social good by bringing together best-in-class tools and technology. Prior to Bonterra, Jill spent 17 years in the nonprofit and public sector leading Information Technology teams, data projects, and performing research.


Sean McSpaden — Moderator, Senior Fellow, Center for Digital Government

Sean McSpaden is an executive level information technology professional with over 30 years of experience in the private, non-profit, and public sectors. His background includes the start-up and management of several small businesses and he has served on the Board of Directors or in Executive Director positions for several 501(c)(3) non-profit organizations. Sean’s public service experience includes progressively responsible positions as an IT analyst, and in statewide coordination, management and leadership positions within the Executive and Legislative branches of Oregon state government. From June 2008 to September 2013, served as the state of Oregon’s Deputy State Chief Information Officer.
